Social forms are now due in, however there were a couple of glitches in the form in the download page of this site. Firstly they have the wrong email address on the form. It should be IDORSocial@gmail.com without the underscore. Secondly there is no column for the dessert! So if you want the dessert of your choice can you update the form with the dessert in the “other dietary requirements” box and resend it (to IDORSocial@gmail.com).
